Clinical Coordinator Home Dialysis RN

Great opportunity for a Registered Nurse with HH, PD and management experience. This is a career opportunity for a dialysis nurse looking to advance to nurse management.

ESSENTIAL DUTIES AND RESPONSIBILITIES YOU WILL LEARN:

• Provide nursing care to PD and home dialysis patients.
• Responsible for Plan of Care, monitoring, administration, and documentation of medication administration.
• Oversight of HH/PD Staff (including Certified Clinical Hemodialysis Technicians, and CCHTs in training).
• Communication with the patient’s nephrologist.
• Communication with other providers (SNF, Acute Care, Home Health) as required for the clinic’s patients.
• Work with other members of the Inter-Disciplinary Team on a regular basis (Social Worker, Dietitian, Nephrologist, CCHT).
• Upholds and supports company values in words and actions. Speaks respectfully to patients and teammates and strives to follow company policies and procedures, asking when unclear.

MINIMUM QUALIFICATIONS:

• California Registered Nurse (RN) required.
• 2 years minimum experience in home dialysis a must.
• 2 years minimum experience in a management role.
• Strong computer literacy and Electronic Medical Record (EMR) experience required.
• Must be a team player and work in an environment with a flexible schedule.
• Ability to interact with all levels of the interdisciplinary team.
• Ability to work in a fast-paced environment with changing priorities.
• Desire to grow and develop in the dialysis profession.
